We are upgrading our office software to better serve you.
As we transition, we are required to gather new complete information when you check in, so please plan on arriving 15 minutes prior to your scheduled appointment time to allow for the completion of this paperwork.

2011/12 Flu Season
We, along with the CDC, recommend flu vaccine for all of our patients over 6 months of age.
People with chronic illnesses such as asthma and diabetes and heart disease are at higher risk of complications from the flu and should call early to ensure that they receive a vaccine.
We have already recieved shipments of both the injectable (shot) and intranasal (mist) forms of the vaccine.
Please call the office to schedule your flu vaccine as soon as possible!
