Our courteous office staff works hard to ensure patient comfort and convenience.  To help you better understand our office operations, we have included the information you need about our hours, locations, appointments, insurance acceptance and billing below.

Walk-In Hour

During the COVID-19 pandemic, we will not be having walk-in hours, and all visits will be by appointment only. 

Insurance and Billing

Please call us for information specific to your insurance company and plan.

Payment Options

Cash, check, credit or debit cards.

Copays

Our contract with your health insurance company dictates that we must collect a copay at each visit. You may have to reschedule your visit if you do not have your copay at the time of the visit.

No Show Policy

If you do not come to a scheduled appointment or do not cancel that appointment with at least 24 hours notice, you may be charged a $25 missed appointment fee.

Multiple missed appointments may result in discharge from the practice.
